I teach high school math but here are a couple examples that might give you some ideas
https://goformative.com/clone/RVPUKA this on has embedded videos, an interactive applet and multiple choice / show work question - you could embed a video to play and ask kids to draw picture of what they learned
https://goformative.com/clone/CRVZYS - this one is a three act math play where students can type, draw, or record their predictions, information and conclusions
